Can you get medicare in new hampshire if you had a kidney transplant out of the united states?

I had a kidney transplant out of the united states, now i just came back in the states and i don't have any insurance or medicare coverage,so now i gotta get my blood tests done once every 3 months and i cannot afford it because blood tests are too expensive, so my question is can i get medicare in new hampshire even if i had my kidney transplant out of the united states?
